Initial assessment for a motor vehicle related accident as a pedestrian, driver, passenger, or cyclist. Provide your accident benefits assigned claim number (provided by the car insurances AB Adjuster after speaking with them to open your claim to receive care) or policy number. This is required to process appointment fees via direct billing. *car accident claims offer direct billing over 12 weeks+ care. Treatment may be initiated, further diagnostic imaging according to condition. (FYI: you can select your preferred chiropractor, your car insurance has no say over this.)

Motor Vehicle Accident Assessment

New patients only (up to 1 hr)

1 area of the body (i.e.: neck pain OR lower back pain etc). Areas directly above and below are screened, for example, if your main concern is neck pain, I also assess your head/shoulder area and upper back/upper thoracic spine. Included posture screen.

Options: more than 1 area of concern (i.e.: Neck, lower back, knee...), please select the Extended Initial option when booking in (1.5hrs appointment $200 rate. Different form will be emailed upon booking) OR any other areas of concern can be assessed upon your follow-up appointment by selecting “new complaint”

Included: 
History taking, physical assessment/posture screen, diagnosis, plan of management and introductory treatment. A safe space that is patient-centered, inclusive, and judgement free. You will receive an emailed initial intake form (takes 5-10mins) to complete prior to your appointment.

Chiropractic Initial Visit

New patients only 

(Up to 1.5hr)

1-3 areas of the body

Included: History taking, physical assessment/posture screen, diagnosis, plan of management and introductory treatment. A safe space that is patient-centered, inclusive, and judgement free. You will receive an emailed initial intake form (takes 5-10mins) to complete prior to your appointment.

Chiropractic Initial visit - Extended

Treatment options available: 

15 minutes: (Express visit): Chiropractic mobilization/manipulation, minimal soft tissue therapy. 
(If you have a new issue or reaggravated complaint, consider booking a longer appointment i.e.: new condition or a longer subsequent visit.)

30 minutes: Chiropractic mobilizations/manipulations, active/passive muscle release therapy, Graston technique, acupuncture/taping/rehabilitation review.

60 minutes: chiropractic mobilization, manipulation, Graston technique, active/passive muscle release therapy/acupuncture/taping/rehab review, progressive muscle relaxation (PMR)/mindful meditation, nutrition/supplement review, diagnostic imaging review.

Chiropractic Subsequent Visit

Options:
Contemporary Medical Acupuncture: acupuncture, electro-acupuncture, acupressure; chiropractic care, soft-tissue therapy (muscle release therapy/Graston technique) *acupuncture provided is still under Chiropractic for insurances purposes*

Chiropractic/Acupuncture  subsequent visit

By selecting pre-paid visits, you agree to purchase the 5 visit package of $380 whereby each visit is $76 and an itemized receipt is provided after each visit or statement post package. 

Treatment options available: 

30 minutes: Chiropractic mobilizations/manipulations, active/passive muscle release therapy, Graston technique, acupuncture/taping/rehabilitation review.

Chiropractic subsequent 30mins pre-paid

By selecting pre-paid visits, you agree to purchase the 5 visit package of $261.25 whereby each visit is $52.25 and an itemized receipt is provided after each visit or statement post package. 

Treatment options available: 

15 minutes: (Express visit): Chiropractic mobilization/manipulation, minimal soft tissue therapy. 
(If you have a new issue or reaggravated complaint, consider booking a longer appointment i.e.: new condition or a longer subsequent visit.)

Chiropractic subsequent 15min pre-paid

New pain or injury assessment, diagnosis, and treatment.

New Condition

Reassessment of existing condition that has been treated with Dr. Maria, review of existing treatment plan, diagnosis, and treatment or referral for imaging if required.

Re-assessment

New patients require an initial visit before an orthotic appointment (can be booked together). Please select both the initial chiropractic visit option and add a note “plus custom orthotic appointment” so your appointment is back to back without a buffer. If you are able to select the orthotic appointment as well, please do so and we will adjust the buffer accordingly. 

Existing patients just select custom orthotic visit. What’s included in the orthotics appointment: Biomechanical assessment, 3D foot scan, custom orthotic pick-up and follow-up.

*Optional: Second pair (of equal value) purchased at same time is $450*

Custom Orthotics 1 pair

Please bring your shoes to try the orthotics for fitting. Orthotics along with order documents are only dispensed to the patient at pick up time. Please arrange a day of pick up during the week or if weekend preferred, on a Saturday. Please be on time for your orthotic pick-up as additional time is subject to fees (additional 15mins = $53). If you miss your appointment, you will be required to reschedule and are subject to missed appointment time fees if missed again.
