Chiropractic Initial Visit

Description

New patients only (up to 1 hr) 1 area of the body (i.e.: neck pain OR lower back pain etc). Areas directly above and below are screened, for example, if your main concern is neck pain, I also assess your head/shoulder area and upper back/upper thoracic spine. Included posture screen. Options: more than 1 area of concern (i.e.: Neck, lower back, knee...), please select the Extended Initial option when booking in (1.5hrs appointment $200 rate. Different form will be emailed upon booking) OR any other areas of concern can be assessed upon your follow-up appointment by selecting “new complaint” Included: History taking, physical assessment/posture screen, diagnosis, plan of management and introductory treatment. A safe space that is patient-centered, inclusive, and judgement free. You will receive an emailed initial intake form (takes 5-10mins) to complete prior to your appointment. Seasonal reminders: Masks are optional for patients (if you'd like to wear a mask, please bring your own.) **Seasonal safety precautions: please do not book in if you have any flu symptoms.
